2. Confirmation of Referral
   Once we receive your referral, we will promptly confirm receipt. We aim to respond within one business day to acknowledge that we have begun processing your referral.
3. Patient Assessment Form
   After confirmation, we will provide an assessment form to gather all necessary patient details and medical history. This ensures we are fully prepared for the initial consultation.
4. Notification to GP and Patient
   We will send a formal notification to the patient’s General Practitioner (GP) to inform them of the referral and any necessary details. Additionally, we will send an information packet directly to the patient with details about their upcoming consultation, what to expect, and any preparatory steps they need to take.
5. Appointment Scheduling
   Our patient care team will contact the patient to schedule their consultation. We offer flexible scheduling to accommodate patient needs.
6. Keeping You Informed
   We believe in collaborative care. You will be kept informed throughout the patient’s journey with us. After the initial consultation, we will update you on the treatment plan.
7. Treatment Updates
   Following the patient’s treatment, we will provide you with an update on the outcome and any follow-up care planned.
8. Continuous Communication
   Should there be any changes or updates in the patient’s care plan, we will communicate these to you promptly. We also encourage open communication should you have questions or require further details at any point.
9. Feedback and Follow-Up
   Post-treatment, we will share feedback from the patient regarding their experience and outcomes and direct them back to your care. We value your partnership and are committed to ensuring that the care your patient receives at Sapphire Eye Care meets the highest standards.
10. Completion of Care Cycle
    Once this patient’s treatment is complete, we will send a final report to both you and the patient’s GP, closing the loop and ensuring all parties are informed of the patient’s care outcome.
