The vast majority of macular holes are still amenable to treatment, even if they have been present for many months or even years. Of course, the prognosis, the likelihood of getting good vision and macular hole closure, goes down with time, and certainly after a year or two, you would have to have a fairly comprehensive discussion as to what the likely outcome was with surgery and whether the risk and inconvenience was justified.
